Forms/Waivers

A member waiver must be sent with membership each year.

Competitors who are not members of SADA must sign a non-member waiver and send with entry.

Competitors who are not members of SADA or ESA (full or associate) must also pay a $20.00 day membership and complete a non-member waiver form and send with entry - or join our club for $35.00 for the whole year.
